Eleventy renders Nunjucks, Liquid, and plain HTML templates to static HTML, and the form is plain HTML, so it drops into whichever you use. Add it to a page template or a layout you share across the site.

Put the form in an include and pull it into pages, or register a shortcode in .eleventy.js so you can drop the form anywhere with a single tag. Either keeps one copy of the markup.

Nothing to configure
