HTML survey form

Build an HTML survey form with no backend. Use dropdowns for quick answers and one open comment field, copy the markup, and responses land in your inbox - no server and no JavaScript required.

How to build it

  1. Create a form in your Formpaste dashboard and copy its access key.
  2. Copy the HTML above and paste it into your page.
  3. Replace YOUR_ACCESS_KEY_HERE with your access key, then publish.

Give me a plain HTML survey form with a couple of select dropdowns and one textarea that POSTs to https://api.formpaste.com/submit with a hidden access_key input. No JavaScript, no backend.

Or connect the Formpaste MCP server to Claude Code, Codex, Cursor, Claude Desktop, VS Code, or any mcp-remote client. See the setup guide.

HTML gotchas

A plain HTML form needs no framework and no JavaScript. Point the form's action at Formpaste, add a hidden access key, and the browser posts it for you. This is the simplest possible setup and works on any host.

No JavaScript required

A form with action set to the Formpaste endpoint and method POST submits on its own. You only add JavaScript if you want to stay on the page and show your own success message instead of a redirect.

Use a redirect for the thank-you page

Set a redirect URL in your form's dashboard settings and the browser lands the visitor on your thank-you page after posting. No script needed.

The hidden access_key wires it up

Every form needs the hidden access_key input. It tells Formpaste which form the submission belongs to. Keep it in the markup exactly as generated.

Style it however you like

The markup is yours, so any CSS works. Switch the styling toggle to CSS or Tailwind for a starting point, or add your own classes.

What to get right

Keep it short to keep completion high

Every extra question loses responses. A couple of dropdowns and one open comment field usually gathers what you need without fatigue.

Dropdowns beat free text for analysis

Fixed choices (select fields) are easy to count later. Reserve the textarea for the one open-ended question that genuinely needs it.

Make email optional for honest answers

Anonymous responses are often more candid. Ask for email only if you need to follow up, and mark it optional.

How it works

Three simple steps for form submissions to land in your email inbox.
Create your form, set your form's action URL to Formpaste's endpoint, and add your access key.

1

Create your form

Sign up, verify your email, and create an access key in your dashboard.

2

Copy the code

Paste the snippet above into your project. Pick CSS or Tailwind to match your site.
