This template ships with the fields a real booking needs and nothing extra. Name and email are required, so you always know who is asking and how to reach them. A preferred date field takes a plain YYYY-MM-DD value and a preferred time field takes something readable like “2pm”, both required, so every request arrives with a concrete slot rather than a vague “sometime next week.” A free-text notes textarea rounds it out, giving people room to mention what the appointment is for, who is coming, or anything you should know in advance.
