What is a wholesale order form?
A wholesale order form is how a retailer, stockist, or reseller asks to buy from you in bulk. It is business to business, so the request looks different from a retail order: you need to know which company is asking, who the person behind the request is, and what they want in what quantity, before you can price it. The output is a clear email that gives your sales side everything it needs to prepare a quote and an invoice.
Be clear on what this is not. A wholesale form collects the request, it does not run a checkout. Trade pricing usually depends on volume, account status, and terms you set per customer, so the right flow is to gather the details, quote them, and follow up with an invoice or a payment link rather than charging a card in the form.
When to use it
Reach for a wholesale order form whenever your buyers are businesses rather than individuals. A brand selling into independent shops, a maker supplying cafes and boutiques, a distributor taking reseller orders, or any operation that sets minimum order quantities and negotiated pricing fits this shape.
It is the right tool when each order needs a human step: checking trade eligibility, confirming stock at volume, applying an account discount, or agreeing payment terms. If you have ever wanted a clean way for a stockist to say “here is my shop, here is what I want, please quote me,” without building a gated B2B portal, this is the form for that.
What to include
This template asks for exactly what a trade request needs. Business name is required and comes first, because in wholesale the account is the company, not just the person. Email and contact name are required too, so you know who to reply to and who you are dealing with inside that business.
Product or SKU and quantity are both required, since a quote depends on knowing precisely what and how much. Quantity is a proper number field, which lets buyers enter a real count and lets you validate a sensible minimum order right in the browser; the generated code sets type=number for you. The order notes textarea is optional and gives buyers room to add delivery timing, a resale certificate, or account details without bloating the form for everyone else.
Tips for winning wholesale accounts
Route it to the right desk. Wholesale is not a support query, so forward these submissions to whoever handles trade, with reply-to set to the buyer. A quote then becomes a single reply straight from your inbox.
State your terms near the form. Minimum order quantity, lead times, and how trade pricing works belong right next to the fields, so a request arrives already matching what you can offer instead of triggering a round of clarification.
Turn a quote around fast. A stockist deciding what to carry is comparing suppliers. Reply quickly, even just to confirm you have the request and are pricing it, and you signal that you are easy to buy from at volume.
